    Will coordinate services and provide telephone support for patients and carers as required. It will need to be able to:

    — operate 24/7 365 days per year,

    — be accessed by any health professional (acute trust, hospice, rapid response, GP and DN) as well as patients and carers 18,

    — book the required service cover such as low level support,

    — activate the rapid response service,

    — activate transfer to a community bed (including hospice) coordinated with the managers in that service, as advised by health care professional (GP, DN etc),

    — activate transfer to acute hospital as advised by health care professional,

    — inform the patient / carer of services that are booked for them including the expected response time. This must include the name of individual who will be making the home visit,

    — inform the health care team (GP, DN etc) of the services booked for patients and the expected response time (this may be retrospective),

    — give patient and carer advice which may include sign posting to other areas of care such as welfare rights,

    — take enquiry from patient or carer about care and refer to the appropriate service (e.g. equipment requirement passed to DN service to action),

    — make follow-up phone call to patient and carer to provide information about expected action from their enquiry above (e.g. DN will access equipment need and have delivered by x date and time).

    Transfer from acute hospital or community bed to alternative care environment should be organised from that service as part of patient overall discharge planning.

    This contract will be awarded as a Part B Service with a possible extension of 1 year, + 1 year, subject to satisfactory performance through the initial contracted stage.

    If and when this requirement is offered to tender, this may be done in whole or in part via electronic means using the Internet. The contract is for the benefit of other participating NHS bodies (whether acting individually, or on behalf of, or together as members of any consortia) along with any other non-NHS bodies which the participants deem necessary for the delivery of services or goods to NHS bodies, local authorities, other government departments or non departmental public bodies charged with the delivery of healthcare or health related services. The selected supplier(s) will be expected to provide a Coordination Centre services (as specified within the tender) from within their range as and when required by re:source CPH and its member Trusts from time to time. Future additions to their ranges may also be included at a later stage.
