Hankinta koskee Keski-Pohjanmaan maakunnassa Kokkolan ja Kruunupyyn kunnan alueella sijaitsevien tehostetun palveluasumisen, senioriasumisen ja kotihoidon yksiköiden ympärivuorokautista etälääketieteellistä vastuulääkäripalvelua siten, että asukkaat hoidetaan ensisijaisesti heidän omissa, tutuissa asumisyksiköissään tai asiakkaan omassa kodissa.

Description of the procurement:

Palvelun tulee muodostua seuraavista osista:

A. Potilaiden lähtötilanteen kartoitus;

B. Vastuulääkäri: jokaisen potilaan osalta puhelinkierto kerran viikossa juoksevien asioiden hoitamiseksi ja jokaisen potilaan kohdalla kierto yksiköissä 4 kertaa vuodessa;

C. Geriatrin osallistuminen potilaiden lähtötilanteen kartoitukseen ja hoitosuunnitelman laatimiseen;

D. Ympärivuorokautinen puhelinpäivystys joka päivä (24 h / 7 pv);

E. Pikadiagnostiikka: jokaiseen yksikköön sijoitetaan pikadiagnostiikkalaitteet, sekä potilastietojärjestelmän käyttö;

F. Oheistoiminta.
