This PIN notice relates to a call for competition for Specialist Stroke Support Services.
The service provides specialist support for Stroke Survivors their families and carers resident in Slough. The main outcome is to reduce the risk of future episodes of stroke through increased awareness and understanding, and support Stroke Survivors, their families and carers to adjust to the potential changes that have occurred related to the stroke.
Slough Borough Council expects the Provider to continue to develop the service to ensure it meets the needs of the current users and be adaptable to meet any future change. The latter may include the use of technology to offer a range of accessible options for support.
The Provider must be able to demonstrate how they have contributed to Social Return on Investment including supporting people to maintain their independence to delay the need for care and or reduce stroke related hospital re-admissions. The Provider must be able to demonstrate they have robust links or can develop these with other agencies including health Stroke services.
The contract is likely to be for 2 years with an option to extend for an additional year.
The procurement process will be undertaken on the Dynamic Purchasing System early 2017.
Estimated cost excluding VAT:
