Description

Southwark Council’s vision for carers is that through identification, advice and assessment, carers will be supported to maintain their caring roles within the community. If eligible for services the carer can be supported to identify their needs and where
such assistance is necessary to put support in place to maintain their caring role. The service would be able to offer a range of information and advice on Southwark Council processes, third sector services, provide advocacy and assist the carer to recognise
how they can improve their current situation. The outreach service will be an all age service, particularly focusing on hidden and hard to reach carers. The emphasis for each service will be on the following groups:- • Carers at the early stage of their role.
• Carers who are hard to reach (for example BAME/working carers/young carers) • Carers with complex caring situations (caring for fluctuating conditions and caring for more than 50 hours per week) • Carers who are caring for people with dementia and/or long
term conditions
