County Durham and Darlington Foundation Trust wishes to commission a service for the provision of patient centred domiciliary care for adults in the Sedgefield and Dales Localities. The providers need to deliver a responsive high quality and efficient
service for clients receiving (RIACT) multi-disciplinary intervention following a health crisis to enable them to regain/improve their levels of functioning and independence The purpose of the RIACT Service is to provide assessment, rehabilitation, clinical
intervention and support to patients in their home setting. The care provider will be expected to provide responsive flexible care using individualised careplans which are goal orientated and outcome focused and are continuously adapted to meet the changing
needs of the client
