Description

Staffordshire County Council wishes to invite tenders for the provision of Intermediate Care for Older People for residents in the Uttoxeter and surrounding areas of the East Staffordshire district of Staffordshire. The two (2) beds which the Council and
the PCT wishes to contract for must be located in single rooms within registered care homes, being situated in Uttoxeter or immediate surrounding area, providing up to two (2) beds. It is a fundamental condition of the contract that the successful tenderer(s)
is appropriately registered under Part II of the Care Standards Act 2000. The principal requirements of the service are as follows: The care home will provide a level of services and skills that, in conjunction with other community services, aims to enable
Service Users to remain in, or return to, the community. The Service will be used to: (1) Facilitate earlier discharge from hospital following an acute episode of illness; (2) Provide care to prevent inappropriate admission to hospital for people who have
a short-term episode of ill health, which does not require specialist medical intervention; (3) maximise independent living; (4) Reduce the need for long term care The service will enable each Service User to move from dependence to personal independence,
with the focus on the restoration of self-care abilities. Interventions and support will aim to maintain and promote independence and life skills. The Service will provide a time-limited period of intervention for Service Users who do not require the specialist
medical input only available in an acute hospital and who demonstrates an ability to return to their previous or reasonable level of independence. The Service will also re-able Service Users who have been discharged from hospital care, but who lack the skills
and confidence to immediately return independently to their own homes. The Service will be for a maximum period of six weeks and will work in conjunction with East Staffs Community Intervention Service (CIS). The Service will be underpinned by person-centred
individual assessments of each Service User. This assessment will initially be carried out by the CIS and maintained within the service through an agreed package of care/support and ongoing assessments, care-planning, management and evaluation of care.
